I would advise people from downloading and filling out the forms on this application. They want your full name, address, phone number, and YOUR SOCIAL SECURITY NUMBER! There is no reason for them to have that information until you have a winning ticket. You do not give this information out when you go to buy a ticket nor do you need it to check the ticket information at the store you bought it from or on the Massachusetts Lottery website. In addition, at the bottom, they say they will use your Social Security number to check your Experian credit rating. WHY??? Avoid this implication like the plague.
